Registration Information
Cost for care is $8 an hour for first child, $4 an hour for additional siblings.
The following forms must be completed on your first visit:
* Child information card, which can be completed at the center or online!
* Vaccination records from your child's doctor.
We must have vaccination records prior to your child's first stay with us!

**PLEASE NOTE REGISTRATION PROCEDURES**
When you visit our PlayDates for the first time, this information will be reviewed by PlayDates staff before you leave your child with us. At that time, you will need to review and sign several documents, before your family membership is activated. The activation process should take approximately five minutes and MUST be completed in person.
